Creating New Solutions To Problems, Ignoring Maintainig Good Old Ones

Washing hands between patients? Cleaning utensils, e.g., stethoscopes, blood pressure cuffs, X-ray and MRI machines between patients? Focusing on correcting poor practices? Rocket scientists not necessary.
